PADI Open Water Diver certification

A student diver can present the PADI Universal referral for Open Water Diver certification to a instructor. It states that the diver took initial training from another diver training organization. The form directs the diving instructor to complete the student's open water dives, sign a statement, and send the form to the student's original instructor, who should certify the diver.

You can also use the Universal referral form for certification. However, PADI requires you to complete the course within 12 months of your last training. Usually, this is a two or three-day course. During this time you will need to be able to dive together to a maximum depth 60 feet/18m.

Statement for medical purposes

To get certified as a diver, you need to complete the Medical Statement to Universal Referral Form. This form is essential to make sure that you are as healthy and fit as possible before you start diving. This document is designed to protect you underwater. Be sure to answer all questions honestly.


First, verify that your physician signs the forms. This form must be signed by your physician before you begin training to become a scuba diver. If you have a new condition, a medical release will be required. You should also take a photocopy your certification card.

Logbook

Before a diver can dive open water, he or she must complete a medical statement. This usually takes place before a certification course starts. A doctor must sign the release. Changing medical conditions require a new medical release. PADI provides a medical release. The PADI Release Of Liability/Assumption Risk/Non Agency Acknowledgement Form-General Training or the EU version is equivalent to the medical release.

The universal referral form will be different to the one found in a scuba logbook. It will list the open water dives and knowledge-development sections the student has completed. The instructor will sign the document certifying that the student has done the required dives. The student should then return this form to the original instructor who will be able to certify them as certified divers.

The course requirements

Most well-known scuba certification agencies participate in the Universal Referral Program. The Universal Referral Program means that a diver may take the theory or pool portion of an open water course with their local dive center and then do the open water check out dives with another agency. This can only be done in cooperation with a dive centre that is a member of a universal referral programme such as SSI (NAUI), NASDSS or YMCA.

PADI Open Water Divers requires that students pass the Knowledge Development as well as Open Water Dives sections. Additionally, the student diver must provide a medical release signed by his or her physician before diving. To be certified, the student diver must take a photo. Digital images are acceptable.
